The Advance Care Planning website has forms and information brochures for each state/territory. Choose their “Create Your Plan” page and select your state/territory to learn more.

Note that a Heath Direction (called an Advance Directive in some states) allows you to say what treatment you do NOT want if you become unable to make your wishes known.

However a Health Direction does NOT allow you to request voluntary assisted dying (VAD). Although VAD will become available to eligible persons in November 2025, you cannot be approved for VAD if you do not have decision-making capacity.

The former Superannuated Commonwealth Officers Association published a booklet some years ago which you may find useful to fill in, as a record of things your spouse/relative/friend needs to know upon your death.
